It turned out to be a burst abcess. We pulled him out and got him right to our vet's. We believe the wound was the result of a bite that gave Boo Boo FIV. Despite all he's been through, Boo Boo is a heartbreakingly friendly cat. Loves to rub against you and cuddle. He is a real doll.
